<P>PROBLEM TO BE SOLVED: To autograph personal information, to hide the autographed personal information, and to prevent the information from being falsified. <P>SOLUTION: A first plastic film 24 in which a card surface sheet 21 having an autograph area 30 is laminated through a first adhesion layer 23 and a second plastic film 26 which is laminated at a predetermined position on an upper surface of a mounting 10 through a second adhesion layer 27 are subjected to pseudo adhesion. Parts 22a, 22b of a separator sheet 22 are made to intervene in an area corresponding to the autograph area 30 between the card surface sheet 21 and the first adhesion layer 23. A first half-cut 41 from the separator sheet 22 to the first adhesion layer 23 is inserted at a position corresponding to one side of the autograph area 30. A second half-cut 42 from the separator sheet 22 to a pseudo adhesion part 25 is inserted at a position corresponding to other sides surrounding the autograph area 30.
